The Center of Healing Arts – Dr. Dinesh G Patel Psychomotor
Laboratory

The Center of
Healing Arts – Dr. Dinesh G. Patel Psychomotor Laboratory the
first of its kind in India and the Asian Continent was started
at the Paraplegia Hospital, Ahmedabad in December 2001. This
Laboratory was established with the efforts by Dr.Dinesh Patel
one of the pioneers of Arthroscopic Surgery in North America and
obtained 6 workstations with instruments on donation from Smith
& Nephew, USA costing $2,30,132.50 (Rs.1.72 crores). The
Laboratory consists of a full operation theatre set up with all
required arthroscopic instruments and accessories along with
joint models to learn and do a mock surgery on the model. This
set up would enable the students to learn the basic diagnostic
techniques and kick start the basic operative techniques and
transfer the same to an actual orthopaedic surgical theatre on a
patient. Arthroscopic joint surgery is a minimally invasive form
of surgery, which allows the patients to go back to his routine
in a very short time. By promoting the learning of this
technique, ultimately the population at large would benefit
immensely. The surgery is useful in many problems of knee,
shoulder ankle, elbow hip and wrist joints. This set up would go
a long way in helping upcoming Orthopaedic students and surgeons
all over India to learn and master this art. Also theatre
assistants, nursing staff can learn how to handle the sensitive
equipments and assist during actual surgery. Also this
Laboratory activity would encourage such facilities for
endoscopic spine surgery and other branches of surgery like
gynec, thoracoscopy and would add to the welfare of surgeons and
patents population of the country.
At present we are giving hands on
training on knee and shoulder models. We have organised many
hands on workshop with live surgical demonstration to upcoming
orthopedic surgeons and post-graduate students from all over
India.

We are
conducting training programme in the Psychomotor Laboratory
regularly for the post-graduate students of B.J.Medical College,
Ahmedabad and will be planning to give training to P.G. students
and orthopedic surgeons of Gujarat State and from other states.
One Workshop on Hands of training in Arthroscopy was conducted
in April 2002 and second in August 2002 and another workshop on
Hands on Arthroscopy on September 15, 2002 for North Gujarat
Orthopedic Surgeons. A National Workshop on Knee & Shoulder was
organized at the Psychomotor Laboratory on 20-21 February 2003.
Dr.Dinesh Patel from USA was present in the workshop for
delivering lectures and demonstration. Doctors from other parts
of the country are also attending for the hands on training in
batches. Those doctors who wish to get training in the
Laboratory can directly contact the Director of this hospital
for appointment.

A Leg To Stand On Project
A
Leg To Stand On Project was started at the Paraplegia Hospital
on February 2003. This is a joint project by Government of
Gujarat and A Leg To Stand On, New York. This project was
inaugurated by the Hon. Health Minister of Gujarat, Shri
I.K.Jadeja in presence of Secretary to Government Health &
Family Welfare Dept., Shri S.K.Nanda and A Leg To Stand On, New
York team members. During the inaugural function, a cheque of
$25,000 has been handed over to A Leg TO Stand On, Ahmedabad by
the Board of Directors of A Leg To Stand On, New York as initial
donation for starting the project.
A
Leg To Stand On’s mission is to help transform the lives of
children with limb disabilities in Gujarat-India by offering
them to physical capabilities to access the opportunities earned
through education, work and mobility. A Leg or arm handicap can
limit a child’s ability to escape from danger, prevent a child
from getting an education, disqualify a youth from working and
prohibits a child’s ability to play sports.

The
objectives of A Leg to Stand On project is to prepare the level
playing field for handicapped children in Gujarat-India by
efficiently channeling global resources to help correct leg and
arm disabilities and to improve local medical education and
knowledge through the teaching and skill and knowledge transfer
of specialized rehabilitative surgical procedures. Children
below the age of 18 years are given good quality of prosthesis
and orthotis free of cost under this project. A Leg To Stand On
Inc., New York who is sponsoring the project will finance for
the project, according to the requirement of prosthesis/orthosis
and corrective surgery.
A
Leg To Stand On, Ahmedabad has registered as many as 80 children
and provided free good quality prosthesis and orthosis to 25
children.

The
Body will be having linkage with different organizations and
will take up identification, assessment of both preventive/
reversible disability and irreversible disability. To implement
the project smoothly and to get benefit to more and more
disabled children of Gujarat, we seek the co-operation of all
Doctors in the field, Non Government Organizations, Education
Departments, Medical Associations and other philanthropic
institutions working for the welfare of the disabled to
associate with this noble project started with the generous help
of American people.

Approximately 1250 infants are born
every year in Gujarat with one or both clubfeet. Often the
deformity remains undiagnosed, (lack or awareness), is treated
late (late presentation) or is neglected (lack of surgical/other
resources). Children with neglected clubfeet are destined to
grow up with deformed painful feet, physical disability,
dependency, demoralization, depression and despair. In an
agrarian society disability is a major cause of the development
challenges of ill health and poverty, as afflicted individuals
are socially and economically disadvantaged with reduced
educational and employment opportunities. Afflicted females are
further disadvantaged (less likely to marry – more likely to
suffer abuse). The burden of care of the disabled child falls on
the mother, who has less time for other children, domestic,
agricultural and economic activity. The neglected clubfoot
deformity results in disability for the individual, a reduced
standard of living for the entire family, and a burden to the
community.

According to Dr.Shafique P.Pirani,
Associate Professor, Dept. of Orthopaedics, University of
British Columbia, during his visit at Ahmedabad and live
telecast of the Ponseti method of treating the congenital
clubfoot, the possibility of correction of clubfeet without
surgery is an attractive alternative. The Ponseti method of
treating the congenital clubfoot (with accurate manipulation and
casting rather than surgery) was trailed in Uganda in 1999,
after promising Canadian trials. Ugandan results from Makerere
University Dept. of Orthopaedics mirrored the Canadian results
with > 95% correction of deformity without need for surgery. The
goal is to reduce the prevalence of neglected clubfeet in
Gujarat.
The ALTSO Gujarat Clubfoot Project’s
aim is to teach the Ponseti method to doctors in the field and
residents in teaching hospitals and district hospital, who in
turn will then train in the rural areas to the healthcare
workers, prosthetic and orthotic technicians to manufacture foot
abduction braces.

Paraplegia Safari

This
is a noble follow up project started at the Paraplegia Hospital
for follow-up of the discharged paraplegia patients. Safari Team
consists of Orthopaedic Surgeon, Occupational Therapist,
Physiotherapist, Medical Social Worker, and Prosthetic &
Orthotic Technician.
Team
visits all the towns and villages of Gujarat State. It examines
the paraplegic patients who were discharged from Paraplegia
Hospital at their own residence. Their medical, orthopaedic and
rehabilitative care is taken of.
Whole Day Programme:
Treatment programme of Paraplegic patients goes on from down to
dusk. It keeps the patient occupied throughout the day in
performing various treatment and rehabilitation activities.

Other Events

Independence day celebration with active involvement of indoor
paraplegia patients and N.G.Os., sports activities to the
patients, distribution of prizes to the winners of sports and
distribution of vocational aids to the needy patents for their
daily living.
Republic Day celebration with involvement of indoor paraplegia
patients, arranging sports for paraplegia patients, distributing
prizes for winners of sports competition, provides vocational
aids to patients with active involvement of N.G.Os.

Rakshabandan day celebration: All indoor paraplegia patients are
actively participated in the Rakshabandan day celebration with
NGOs. And staff of Paraplegia Hospital.

Identification & Sensitization of
Disabled People in Gujarat

The Paraplegia
Hospital is also working for a Project on " Identification &
sensitization of Disabled People in Gujarat" is association with
Handicap International. Director of Paraplegia hospital is
designated as Co-ordinator for the project and the director and
staff of this hospital is active involved in the project.
A pilot project
for Early Identification and prevention of disability in
Ahmedabad district has been started jointly by Health & Family
Welfare Department and Handicap International (HI). In this
project, it was decided to utilize the available source of FHW/MPWs
who are already trained as field workers and are in constant
touch with rural community and they do regular home visits. MPW/FHWs
being the first person who would come in contact with the
community it was felt necessary to train them fro early
identification of disability and its prevention, so that a
person with disability (PWD) can be identified as early as
possible and can be guided and treated properly to avoid
handicap in future. A Pilot Project for Ahmedabad district ahs
been planned in which 250 workers (approx 50% of the total ) are
targeted to be trained.
FHW/MPWs are
considered as grass root workers who will , upon identifying a
PWD refer the person to PHC/CHC along with a survey form
supplied to them for the same reason. The person will be
diagnosed and given necessary available treatment at the PHC/CHC
i.e the secondary centers, but if the PWD required treatment
which is not available at secondary level then will be referred
to the tertiary center. i.e Civil Hospital , Ahmedabad.
Considering the
above said hierarchy, the training programme in batches, of
FHW.MPWs was started from 22nd April 2003 at State Institute of
Health & Family Welfare , Sola, Ahmedabad. Total 11 training
programmes has been arranged. The faculty members of Medical
College. Ahmedabad in association with Chief District Medical
Officer and Sola Institute arranging this programme.

Pension Scheme for Paraplegia Earthquake victims
A noble scheme for the earthquake
affected paraplegia patients has been introduced by the
Government of Gujarat by way of monthly Pension of Rs.2,000/-
per earth quake paraplegia victims. Total 98 such victims are
regularly receiving monthly pension and they will get this
amount till their death. Those earth quake affected paraplegia
patients left out in this scheme, will also be given the benefit
by submitting all documents of disability and filling the
necessary forms.
All the earthquake affected
paraplegia patients are regularly followed-up by doctors and
physiotherapists in all districts of Bhuj and regular follow-up
diagnostic and operative camps and Paraplegia Safari Programmes
are also carried out by the Paraplegia Hospital staff.
